These challenges are generally not the fault of the processes themselves, or even due to having a process; rather, they are due to the fit between the process and the team. You see this often with technology, designs, and architectures. There are almost no purely wrong technologies: flat network (sure, we can find a use for that), stateful apps (yes, even that can have a purpose), singletons (yes, they have a place in our world).
